The Lakewood Primary Care Patient Online Center will provide
patients with improved access to communicate with LPC in a
convenient manner.
Patients will be able to:
- Update their personal profile including address, phone,
insurance information
- Request an appointment and specify 3 preferences for
day and time
- Request a refill on a prescription
- Download forms to print out and fill out in the comfort
of your home
- Read and send messages in a secure system to contact
the clinic
- Change your password
